I declare that all information submitted by me in this application is true, complete and correct and I understand that if any false information, or omissions, or misrepresentations are discovered, my application may be rejected and, if I am employed, I may be subject to discipline up to and including discharge.
I understand and agree that if I am hired by Working World Inc., my employment relationship with Working world Inc. is at will. This means that just as I have the right to end my employment with Working World Inc. at any time, for any reason , with or without cause and with or without notice. I also understand my failure to report to Working World Inc. for work will indicate I have quit. If I do not report to a Working World Inc office after completing my assignment, Working World Inc may assume that I am no longer ready, willing, able or otherwise available for work?
I agree that I will settle any and all previously unasserted claims, disputes, or controversies arising out or relating to my application or candidacy for employment, and/or cessation of employment with Working World Inc., exclusively by final and binding arbitration before a neutral Arbitrator. By way of example only, such claims include claims under federal, state, and local statutory or common law, such as the Age Discrimination in Employment Act, Title VII of the Civil Rights Act of 1964, as amended grant Working World Inc., including the amendments of the Civil Rights Act of 1991, the Americans with Disabilities Act, the law of contract, and the law of tort.
I grant Working World Inc. the right to investigate the statements made in the application and my suitability for employment. Depending on the position for which I am being considered, I authorize the issuance to Working World Inc. of any credit or consumer report about me including, but not limited to, an investigative consumer report and background check, and release those supplying such information from any liability thereof. I authorize Working World Inc. to release information pertaining to the facts of my employment with this corporation, provided that such facts relate primarily to the periods of employment, the duration of position, my eligibility to be re-hired by Working World Inc., and my pay rate. I also agree to submit to drug screening upon request or as specified in Working World Inc.'s substance abuse policy.
